International Student Admission Guidelines
International Student applicants are nationals of a country other than the Philippines holding a foreign passport and who have been issued a valid Student Visa or Special Study Permit or Permanent Residency Visa by the appropriate Philippine agency. It also includes those who claim dual citizenship.

A student visa or special study permit is required before any international student can officially enroll and the documents to be submitted must be written in English or has an English translation.
1.3 Documents required to get a student visa
1. Duly notarized letter request from applicant.
2. Duly notarized general application form accomplished by the applicant (BI Form NO. MCL-07-001).
3. Original Copy of Notice of Acceptance ( NOA ) containing a clear impression of the school's official dry seal or duly notarized written endorsement from the school for conversion of the applicant's status signed by the School Registrar.
4. Original copy of medical certificate issued by the Bureau of Quarantine and International Health Surveillance or a government medical institution with competence to certify that the applicant is not afflicted with any dangerous, contagious or loathsome disease and is mentally fit.
5. Photocopy of applicant's passport ( bio-page latest admission and authorized stay ).
6. National Intelligence Coordinating Agency (NICA) Clearance.
1.4 Requirements for the issuance of NOA by the University
1. Transcript of Records / Scholastic Records, duly authenticated by the Philippine Embassy or consulate in the applicant's country of origin or legal residence. " SEEN and NOTED" stamp is not acceptable.
2. A notarized Affidavit of Support and proof of adequate financial support to cover expenses for student's accommodation and subsistence, school dues and other incidental expenses. For scholars, certificate of Scholarship from the institution concerned.
3. Authenticated copy of Birth Certificate.
4. Original Copy of the Certificate of Eligibility for Admission (CEA) isssued by the Commission of Higher Education (CHED) for MEDICINE and DENTISTRY students.
5. Enrollment Form / Receipt of Payment.
6. Original copy of the 1998 Revised Personal History Statement (PHS). Duly accomplished and signed by the applicant in English and in national alphabet, accompanied by personal seal if any, with original left and right thumb prints and original photos.
Application For Admission Procedure
1. Submit photocopies ( verified against the original ) of the listed documents ( 1.3 & 1.4 ) to the IEA Office thru the Admission Officer.
2. Interview by the IEA Executive Director or her representative.
3. Assessment of the Scholastic Records / Interview by the College Dean
4. Pay the Foreign Student Admission / Processing fee at the Finance Office.
5. Applicants from non-English speaking country will take the English Assessment Test.
*The result of the English Assessment Test is a requirement prior to enrollment.

6. Take the Admission Tests at the Guidance Office.
7. Verify result with the IEA Office.
8. Applicants who made it will be issued the following letters.

9. Apply for a student visa and I-Card / ACR at Bureau of Immigration and Deportation, Manila.

For Degree Holders Admission
College Of Law
Requirements for Freshmen
The UPHSD College of Law is co-educational and non-sectarian. An applicant for admission must be a holder of a Bachelor of Arts or Science degree from a recognized college or university. The applicant must pass a written admission test and the Dean’s interview.
1. 3 copies of 2x2 colored ID picture and 2 pieces 1x1 colored picture (latest or taken at least 6 months before).
2. Original copy of Good Moral Character Certificate from School Registrar/ Employer/ Brgy. Chairman.
3. Original copy and 2 sets photocopies of Official Transcript of Records with Remarks: “Copy for University of Perpetual Help”.
4. Certified True Copy of College Diploma issued by the School Registrar or Notarized copy of College Diploma.
5. Certified True Copy of High School Diploma issued by the School Registrar or Notarized copy of High School Diploma.

4. Original copy and 2 sets photocopies of Official Transcript of Records (T.O.R.) issued by the School Registrar of the previous law school.
5. Certified True Copy of pre-law Diploma and Official Transcript of Records (T.O.R.).

1. Fill out the application form for entrance examination and secure test permit at the Grade School and High School Information Center.
2. Pay the entrance examination fee at the Cashier’s Office
3. Bring test requirements (long folder, long brown envelope, sharpened pencil, black ballpen) or buy these items in the University Supply Center.
4. Go to the Testing Room at Room 104 of the Grade School Building (GSB)
5. Secure test results from Room 104 of the Grade School Building (GSB)
6. Submit requirements at Rooms 101/102 of GSB
7. Attend an interview at the Principal’s Office
8. Register at Rooms 101/102 of GSB
9. Proceed for Assessment at Window 10 of Business Office
10. Pay fees at the Cashier’s Office
11. Go to Rooms 101/102 of GSB to enlist for sectioning
12. Get list of books and other requirements at the University Supply Center
For High School

For College
1. Fill out the application form for college entrance examination and secure test permit at the Information Center in New College Building (NCB) Room 135
2. Pay the entrance examination fee at the Cashier’s Office
3. Bring test requirements (long folder, long brown envelope, sharpened pencil, black ballpen) or buy these items in University Supply Center.
4. Proceed to the Testing Room in NCB Room 136 to take the test (duration of test is approximately 2 hours)
5. Secure test results from NCB Room 136
6. Proceed for an interview to NCB Room 137
7. Submit requirements to NCB Room 141.
8. Go to the Admissions Office for issuance of Student Number
9. Proceed to the Auditorium to get enrollment form and fill out schedule of classes against the approved schedule.
10. Enrollment adviser approves the accomplished enrollment form.
11. Once approved, student proceeds to assessment at Window 10 of the Accounting Office.
12. Pay the enrollment fees at the Cashier’s Office.
For Graduate School
1. Student proceeds directly to the Graduate School for evaluation of academic records and interview by the Dean
2. If interview is satisfactory, Dean advises student to proceed to the Cashier’s Office to pay for the Admission Examination Fee;
3. Student reports back to Dean to present Official Receipt and secure Examination Schedule;
4. Student takes written admission examination;
5. Secretary of the Dean advises student of the admission test results;
6. Student fills out Graduate School Form 001 and an enrollment form against an approved schedule of subjects for the semester/summer;
7. Student proceeds to the Admissions Office for issuance of Student Number;
8. Student proceeds to Accounting Office for assessment of fees.
9. Student pays the enrollment fee at the Cashier’s Office.
10. Student submits all other requirements to the Dean's Office such as:
