I . Applying for Library Account

Bring your current registration form to the Circulation Staff. Fill out given form and submit to staff, the staff will issue a confirmation stub. Bring the confirmation stub to ICT section to have your picture taken for the Library Profile.

II. Circulation Book

The book collections for circulation are on open shelves. It is arranged according to the Library of Congress Classification (LCC) Schemes.

III. Reserve Books

Books frequently borrowed are placed in this section. The collection also includes collateral reading materials recommended by UPHSD faculty members as required reading for courses and for the shared use of all students enrolled in these courses.

Other Rules & Regulations

• I. Mutilating or Stealing Library Property

  Defacement, mutilation of any library materials or property are strictly prohibited. Any student found doing any of these shall have to replace or pay the current replacement value of the material and pay a fine not less than three hundred pesos. (P300.00) or 50 percent (%) of the current cost of the book, which ever is higher.

• II. Disorderly and Disruptive Behavior

  Any person engaged in disorderly conduct shall be excluded by the librarian from the library premises, and is subject to suspension of library privileges for not more than two weeks.
  
  Disorderly Conduct
  Any person engaged in disorderly conduct such as drunken behavior, creating disorder, tumult, breach of peace, or serious disturbance, gross and deliberate discourtesy, and possession of prohibited drugs.
  
  Disruptive Behavior
  Includes excessive noise, loud discussions, exhibiting acts, etc.
  
  Improper Behavior
  Includes smoking, eating and playing cards within the library premises.

• III. Refusal or Failure to Settle Library Accounts

  Any person who, after due notice, shall refuse or fail without just cause to settle library accounts or obligations shall not be allowed to register, shall not be permitted to use the University Libraries, and shall not be issued a University clearance.

• IV. Group Discussion / Examinations

  Students are cautioned against holding group discussion inside the library as this is distractive to other library users.
  
  Faculty members are also advised against holding of class examinations inside the library premises. This will defeat the purpose of the library being the place for researching. In instances where it is necessary or there would be no other option to use the library for that purpose, prior approval of the librarian must be sought at least two (2) days before the scheduled activity.

• V. Cellular Phones

  To maintain the silence inside the library and to avoid distracting library users, texting and calling inside of the library are STRICTLY PROHIBITED.
